About LAC Transition Services
The Local Area Coordination (LAC) Transition Services is rolling out an important aspect of the National Disability Insurance Scheme (NDIS). The aim of the programme is to empower people with disabilities and their families and carers to build goals, awareness and inclusivity enabling them to direct and control their decisions within their lives.
At Uniting, we believe in taking real steps to make the world a better place. Our not-for-profit services are in the areas of aged care and disability, community services and chaplaincy, We also get involved in social justice and advocacy issues that impact the people we serve. Uniting is seeking a number of Local Area Coordinators (LAC’s) at our Wollongong, Nowra and Gerringong Offices.
LAC’s will be responsible for working with participants, their families and carers, and other parties to encourage change in societal beliefs and structures. They do this by:
Providing support to prospective NDIS participants to engage with the Scheme.
Assisting participants to implement their plans, including building community inclusion and capacity.
Complete plan reviews and full scheme plans for participants.
Are you the person we are looking for?
You will be an active community member who is enthusiastic about supporting and empowering people with disability and their families. We are looking for people who have lived experience, or awareness of the impact of existing or acquired disability upon individuals, families, carers and the community. Qualifications in allied health, disability services, education, human services, community development or social work, and experience in local government or other customer facing environments that support or provide services to the community will be highly regarded. You will also need to be experienced in using computers to input information, as all of our systems for collecting participant information are on-line.
First Round Assessment Centres commence 22nd of November, roles to commence mid January 2017
