Purpose of Position
The Chronic Care Project Officer, Aboriginal Chronic Care Unit (ACCU) will implement the Chronic Disease for Aboriginal People Model of Care to manage the care of Aboriginal people with or at risk of developing a chronic disease. The position will also contribute and support leading and directing the development, implementation, monitoring and evaluation of strategic services or operational programs in Aboriginal Chronic Care. To do this, the position will also supervise and support coordination of Aboriginal health worker initiatives to provide health promotion and education programs appropriate to the needs of the Aboriginal community as well as fostering a multi-disciplinary community health team approach.
The position is also required to work in a collaborative and integrated way with other chronic disease services, including but not limited to: Access and Referral Centre; Cardiac Rehabilitation Unit and Diabetes service etc.

This is a temporary full time leave relief position until 20/10/2017 with the possibility of extension.

Selection Criteria 1. Must be of Aboriginal and/or Torres Strait Islander descent (NB applicant’s race is a genuine occupational qualification and is authorised by Section 14 of the Anti-Discrimination Act 1977, NSW) and have demonstrated knowledge of current Commonwealth, State and Local Aboriginal health priorities and ability to effectively and sensitively liaise and effectively communicate with local Aboriginal and Torres Strait Islander people and communities and agencies.

2. Demonstrated ability to project manage including; successfully plan, lead delivery, evaluate and report on projects within allocated timeframes and resources.

3. Demonstrated ability to engage and work autonomously and effectively under limited supervision with a range of agencies, including working within a multidisciplinary team.

4. Demonstrated high level verbal and written communication skills with an ability to negotiate and work effectively with a range of health professionals/service providers to ensure coordination of care and appropriate allied health and social service referrals.

5. Demonstrated competence in; maintaining accurate and appropriate records; produce routine written correspondence; prepare reports and; present information relevant to preparation of more complex documents, such as ability to research, analyse and collate data.

6. Demonstrated ability to supervise and support Aboriginal Health Workers, develop and evaluate staff education and training programs with a commitment to quality improvement initiatives and ethical practice.

7. A tertiary qualification in a relevant field, or relevant work experience or a combination of study and work experience.

8. Current driver's licence Class C and ability and willingness to travel within the LHD for work purposes
